If you need a home visit, patients are requested to submit an online request form before 10.30am if a home visit is being requested for the same day.

Do you still need an appointment?

Is your request URGENT?

PLEASE SUBMIT YOUR ONLINE FORM BEFORE 10.30am so the doctor can review it as soon as possible.

 

Use the online service to make a request for:

  • An urgent appointment the same day, from 8am until we have reached safe clinical capacity
  • Or a non-urgent routine care appointment between 8am and 6.30pm

 

After safe capacity has been reached, please follow the 'Use the Right Service' information to the best possible care for you, or see Alternative Services
